Community Health Needs Assessment

The Aitkin-Itasca-Koochiching Community Health Board facilitates a Community Health Needs Assessment and Improvement Plan every 5 years. The 2020 assessment and planning began in 2018 when the Aitkin, Itasca, and Koochiching public health departments reviewed health outcomes and health behavior data for the respective county, identify gaps, and followed up with further investigation with their community members.
