About the CHB

The A-I-K CHB is the governing body for local public health in the Aitkin-Itasca-Koochiching Community Health Services area. The mission of the CHB is to lead efforts to prevent, promote and protect the health of people and communities in the tri-county area.

 

About the Position

As part of the tri-county public health team, you’ll work closely with your colleagues and community partners to improve health outcomes within our community. The Public Health Planner will collect, analyze, and use data to improve public health programs and services as well as provide health communications and outreach expertise. This newly created position is designed to improve our local public health agency’s ability to perform foundational public health responsibilities.  

 This position is funded by the Minnesota Department of Health Foundation Public Health Responsibilities grant.

 

Essential Duties Include:

·       Develop population health data dashboard

·       Co-lead a coalition of community partners engaged in the community health improvement planning process in each county. This position will also work to directly implement selected strategies within the community.

·       Contribute to the development of work plans that are based on population health outcome data. 

·       Contribute to the development of work plans that are clearly linked to performance measures

·       Develop and distribute clearly written, well-designed health communications materials for public health partners in a variety of formats, including web-based content, infographics, social media posts, print media, radio content, and professional communications such as educational materials, meeting agendas, emails, etc

·       Grant seeking/writing and implementing public health foundational capabilities as directed.

This is a hybrid eligible position with the expectation that at least 50% of work time will involve working in person with community partners within the three counties (Aitkin-Itasca-Koochiching in Minnesota). Mileage reimbursement is provided from home office. 

 

Minimum Qualifications: 

·       College graduation with a master’s degree in planning, social work, public health, public administration, or a related field, and one year of experience as a social worker, public health nurse, community health educator or related experience with some responsibility for planning or administrative functions, such as coordinating and/or planning community programming for special populations is preferred.  

·       OR  a Bachelor’s degree in a health or human service field and three years of experience as a social worker, public health nurse, community health educator or related experience with some responsibility for planning or administrative functions, such as coordinating and/or planning community programming for specific populations.

·       OR commensurate experience as a social worker, public health nurse, community health educator or similar with some responsibility for planning or administrative functions, such as coordinating and/or planning community programming for specific populations.

·       Preferred candidates will have a strong understanding of public health. 

·       Valid drivers license, reliable transportation, and insurable driver.

2024 Starting Salary: $ 31.25/hr and is negotiable. Benefits include paid time off, health insurance, pension (PERA), paid holidays, bereavement time, and hybrid work
